If you selected that you prefer to participate by mail only on the consent form, or if you are unable to attend one of our Study Centres, a saliva kit will be mailed to you. Your saliva sample is important because it contains DNA (genetic information) and we know that all cancers arise because of changes in DNA. In many cases, the causes of those changes remain unclear. Future research that combines the analysis of DNA with other information you have provided will help researchers understand how and why certain cancers or other health conditions develop.
The package that will be mailed to you will contain:
· Cover letter with detailed instructions
· One sealed Oragene-DNA Saliva Collection kit from DNA Genotek
· One postage-prepaid return envelope
· A plastic bag with a small absorbent pad

INSTRUCTIONS:
1. Follow the instructions for saliva sample collection as provided.
2. Place the collection cup into the plastic bag and seal the bag. Make sure the absorbent pad is in the bag with the saliva collection cup.
3. Write the date you completed the saliva kit on the label located on the outside of the plastic bag.
4. Place the plastic bag back into the return envelope and seal the envelope.
5. Drop the return envelope off at the nearest Canada Post Office or in a red Canada Post mailbox. You don’t have to pay for postage – we’ve already taken care of that. We ask that you provide your saliva sample and return the kit within seven days of receiving it in the mail.
If you do not want to provide the saliva sample, we ask you that you do not open the sealed box with the kit inside. Instead, place it in the pre-paid return envelope provided and mail it back to us. The kits are very expensive and if they remain sealed, they will not be wasted. This will ensure the Tomorrow Project resources are used most efficiently.
